## 1. Research takeaways (product truth)

Sources prioritized: help (`what-is-hra.md`), OS specs (`health-risk-assessment.md`, `wellness-score.md`), admin workforce-health help, dashboard redesign notes for aggregate HRA views. Live marketing was not used as product truth.

### 1.1 What HRA actually is

| Fact | Detail | Source |
|------|--------|--------|
| **Form** | In-app questionnaire (not onboarding, not forced) | Help: what-is-hra |
| **Fields** | Blood group, waist, BP, cholesterol, diabetes, smoking, alcohol, exercise, age/BMI (+ height/weight; prenatal for female users in OS) | Help + OS HRA |
| **Output** | Fitness % 0–100 with categories Excellent (≥85) · Good · Average · Below Average · Critical (<35) | Help |
| **Honesty line** | **Informational, not a medical diagnosis** | Help |
| **Wellness Score** | Feeds **Baseline 20%**; without HRA baseline defaults to **50** | Help + wellness-score OS |
| **Scoring weights (baseline factors)** | Metabolic 50% · Habits 30% · Exercise 20%; missing factors re-weighted | OS HRA / wellness-score |
| **HR visibility** | **Aggregate** risk distribution; individual HRA private to employee | OS marketing brief + dashboard notes |
| **Lite Mode** | HRA **not available** | Help |
| **Retake** | Allowed when health situation changes | Help |
| **Completion** | Voluntary; help notes ~20% complete historically (use carefully, not as a fabricated KPI) | Help |
| **Targeting** | Tag-based challenge audiences by risk factor (e.g. high BMI, sedentary) | OS HRA |
| **Labs** | Lab reports / biomarkers complementary, aggregate path for annual check-ups | Shared research |

### 1.2 Role in Solutions IA (data chain)

```
HRA (Data in)  →  Workforce health insights (Data out)  →  Wellness rewards (Action)
                         ↑
              Org Wellness Score: Baseline 20% + Participation 30%
              + Activity 30% + Adherence 20%
```

This page owns **baseline screening and privacy**. Insights owns score dashboards and AI observations. Rewards owns points and redemption. Do not dump the full insights product here.

### 1.3 What HR buyers care about (objections)

1. **Can I see who is high-risk?** → No individual HRA; aggregate only.
2. **Is this medical / liability?** → Informational tool, not diagnosis; employees directed to clinicians for concerns.
3. **Will employees fill it out?** → Voluntary, private, improves personal Wellness Score accuracy; not forced onboarding.
4. **What do I do with the data?** → Risk distribution informs program design; tag-based challenge targeting; baseline accuracy for Org Wellness Score.
5. **Is it compliant?** → HIPAA alignment, SOC 2, GDPR, ISO 27001/27701 (same enterprise claims as other pages).

### 1.4 Proof

Optional / light only. Named BMI outcomes (Tata avg BMI 24, Brazosport 30→~27) are program-level, not pure HRA-page proof. Prefer **omit full customer-result section** and rely on product honesty + chain to insights. If a strip is needed: frame as “baseline feeds score accuracy,” not invented completion rates.

### 1.5 Assumptions

- Buyer is HR evaluating a wellness stack, not a clinician buying EHR screening.
- Page must not sound like clinical surveillance.
- Primary conversion: Book a demo.
- Sibling links use relative mock filenames from shared research.
- No em-dashes. Sentence-case. Verb-led CTAs.

## 2. Page strategy

### Job of this page

Help an enterprise HR buyer answer:
1. What baseline do we get without turning wellness into a medical program?
2. What does the employee experience look like (questionnaire → score)?
3. What does HR **control / see** (aggregate only)?
4. How does privacy work, and will Legal / IT accept it?
5. How does HRA connect to Org Wellness Score and the rest of the chain?
→ Book a demo.

### Positioning line

> Health Risk Assessment is the voluntary baseline that makes workforce wellness measurable: employees get a private fitness percentage; HR gets aggregate risk patterns and a truer Org Wellness Score, never individual answers.

### Narrative arc (distinct from challenge pages)

| # | Section | Intent |
|---|---------|--------|
| 1 | **Hero** | Data-in promise; privacy-first; product-real HRA result phone + aggregate HR float |
| 2 | **Problem** | Programs without baseline fly blind; default baseline of 50 is neutral, not real |
| 3 | **How it works** | Employee flow: Profile → questionnaire (nine areas) → fitness % + category |
| 4 | **What the score means** | Categories Excellent → Critical; informational, not diagnosis |
| 5 | **What HR sees** | Aggregate risk distribution, completion coverage, challenge tagging; no individual HRA |
| 6 | **Privacy objections** | Explicit privacy contract; Lite Mode / voluntary / retake honesty |
| 7 | **Chain forward** | Feeds Baseline 20% → Insights → rewards / challenges |
| 8 | **FAQ** | Real objections (~4–5) |
| 9 | **Trust + final CTA** | Compliance strip + Book a demo |

### What we deliberately de-emphasize

- Challenge formats, leaderboards, fraud integrity (wrong archetype).
- Fabricated completion KPIs or clinical ROI.
- Claiming HRA is onboarding or mandatory.
- Dumping full Workforce Health dashboard feature list.
- Marketplace / gift cards (rewards page).
- Lab report deep dive (complementary only, one line).

### S1 — Hero

- **Eyebrow:** Solutions · Health Risk Assessment · Data in
- **H1:** Baseline workforce health without turning wellness into surveillance.
- **Lead:** A voluntary in-app assessment captures the numbers that matter. Employees get a private fitness percentage. HR sees aggregate risk patterns that feed the Org Wellness Score.
- **Primary CTA:** Book a demo
- **Secondary CTA:** See pricing
- **Micro:** Individual answers stay private · Aggregate risk for HR · Feeds Baseline 20%

**Hero visual:** Phone mock of HRA result (fitness %, category, sample factor bars) + floating admin card “Workforce risk · Aggregate only” (illustrative distribution). Label illustrative.

### S2 — Problem

- **Eyebrow:** Why baseline matters
- **H2:** Challenges without a health baseline are half a story.
- **Lead:** Step counts and challenge join rates show effort. They do not show whether the workforce is starting from a healthy foundation.
- **Three tension cards:**
  1. **Neutral default** — Without HRA, baseline defaults to 50. Your Org Wellness Score is incomplete.
  2. **Blind targeting** — Programs stay one-size-fits-all when risk factors are unknown.
  3. **Trust barrier** — Employees will not share health data if HR can open individual files.

### S3 — How it works (employee)

- **Eyebrow:** Employee experience
- **H2:** A short questionnaire. A clear fitness percentage.
- **Steps:**
  1. Open Profile → Health Risk Assessment
  2. Answer nine health areas honestly
  3. See fitness % and category
  4. Retake when health changes
- **Nine areas (chips or compact grid):** Blood group · Waist · Blood pressure · Cholesterol · Diabetes · Smoking · Alcohol · Exercise · Age & BMI
- **Note:** Not part of onboarding. Not available in Lite Mode. Voluntary.

### S4 — Score meaning

- **Eyebrow:** What employees see
- **H2:** Categories that guide attention, not diagnose disease.
- **Table/cards:** Excellent ≥85 · Good 70–84 · Average 50–69 · Below average 35–49 · Critical <35
- **Callout:** Informational tool. For health concerns, employees consult their clinician.

### S5 — What HR controls / sees

- **Eyebrow:** For HR & benefits leaders
- **H2:** Aggregate risk you can act on. Individual answers you cannot open.
- **HR outcomes:**
  - Risk distribution across metabolic, habits, and lifestyle factors (aggregate)
  - Coverage: who completed vs still on default baseline (population view)
  - Tag-based challenge audiences by risk factor for targeted programs
  - Cleaner Baseline component on Org Wellness Score
- **Admin mock:** Illustrative donut / bars for risk bands + privacy note “n suppressed under cohort minimum”
- **Explicit line:** Individual HRA responses are private to the employee.

### S6 — Privacy contract

- **Eyebrow:** Privacy & trust
- **H2:** Built so Legal and employees can both say yes.
- **Points:**
  - Individual HRA private; HR sees aggregates
  - Not for performance management
  - Informational, not medical diagnosis
  - Voluntary completion; retake supported
  - Enterprise posture: HIPAA · SOC 2 · GDPR · ISO 27001 · ISO 27701

### S7 — Chain forward

- **Eyebrow:** Data in → data out → action
- **H2:** HRA is the first link in a measurable wellness system.
- **Three chain cards:**
  1. **Health Risk Assessment** (you are here) — baseline data in
  2. **Workforce health insights** — Org Wellness Score, trends, AI insights
  3. **Wellness rewards** — points and redemption tied to real effort

1. Is the HRA a medical diagnosis?
2. Can HR see individual employee answers?
3. What if employees skip it?
4. How does HRA affect the Org Wellness Score?
5. Can employees retake the assessment?

## 4. Mega-menu fit

| Column | Entry | Role |
|--------|-------|------|
| Wellness challenges | Library + formats | Programs people join |
| **Workforce health & rewards** | **HRA (Data in)** | Baseline screening |
| | Insights (Data out) | Measure and act on signals |

HRA is **not** a challenge format page. Mega-menu copy: “Voluntary baseline screening that feeds your Org Wellness Score.”

---

## 5. Design decisions (pre-build)

1. **Archetype = conversion / trust**, not formats explorer. Privacy and aggregate HR view get equal visual weight to the phone mock.
2. **Product-real HRA UI** (result screen + factor breakdown), not stock “clipboard” illustration.
3. **Chain strip** replaces customer-logo proof so the page teaches IA without inventing metrics.
4. **No em-dashes; lean copy.** One-line cards where possible.
5. **Illustrative labels** on all mock numbers.

## 7. Gaps / open questions

- Live marketing page for HRA may be thin or missing; this mock is product-first.
- Exact admin UI for HRA aggregates is still evolving (dashboard redesign); mock is **illustrative**, not pixel-locked to a shipped screen.
- Help notes ~20% historical completion; we do **not** lead with that as a marketing KPI.
- Cohort suppression minimum (e.g. n=5) appears in dashboard design notes; mention carefully as privacy practice if shown, labeled illustrative.
- Lab reports are complementary; not fully productized on this page.
- Sibling Insights / Rewards / Platform / Hub pages may not exist yet; relative links still ship for IA coherence.
