The health risk assessment is a deeper, separate questionnaire. Finishing it returns a personal 0 to 100% fitness score, a status across seven bands from Critical to Excellent, and tips for the employee. It unlocks nothing and gates nothing: calorie targets come from the basic profile, and no one is penalized for skipping.
